What are requirements for a breast reduction?

Okay so I am 14 years old. In January of 2011 I was a c cup and by the end of march I was a dd. I have sharp pains in my back, shoulders and neck. Also people have made up mean nicknames about it. My mom thinks breast reduction is a good idea. Would I be a candidate?
